After a Care Team member enrolls a patient in the health monitoring program, a kit will be sent out to the address added to the patient's profile. The kit can include different configurations of devices, depending on their program or health condition. Kits can also be customized with large or small blood pressure devices and an optional bariatric scale.
When a patient begins their program, they will receive a call from the support team to assist them with unpacking and setting up their kit. They also have a user guide in their kit and help videos on the tablet that describe the process. Alternatively, if configured in the portal and required by the Care Team, the patient can be set up for assisted installation. Assisted installation means that a support technician will go to the patient's location and help the patient begin their program.
Accepting Terms and Conditions
After the introduction video has played, the patient must read and select I AGREE to accept the End User License Agreement (EULA).
Note: The patient will not be able to proceed until they have agreed to the program’s terms and conditions.
Logging In and Creating a Password
When the log in screen appears, the patient will create their password. Using the keypad, they can choose a four-digit number that is easy to remember, for example their birth year (for example, ‘1942’). They cannot use one number four times (1111), nor can they use sequential numbers (1234).
Remind the patient to write this number down and store it in a safe place as they will need to reference it later.
