How To Monitor Patient Biometrics Follow

The Patient Monitoring page is the central location for viewing a patient's progress with their pathways and biometric data.
The Program Trend area at the top of the page shows a chart of the patient's health score over the time they have been participating in the health monitoring program. The colors coordinate with the level of their health score, as follows:
- Scores of 0 to 50 (including 50) are red
- Scores of 51 to 75 (including 75) are yellow
- Scores greater than 75 are green or gray
When you open the Patient Monitoring page, data for the most recent 14-day period appears in the charts. In the Program Trend area, you can click any portion of the bar chart to populate the Biometrics and Health Index for that particular 14-day period in the charts below. This is helpful if you are looking for an alert that happened during a previous time period than what is currently displayed.
The Health Index provides the health scores for the patient over a 14-day period along with the date of the score. When you move the focused time period in the Program Trend area, the Health Index area is updated with patient scores from the highlighted time period.
The Biometrics Data chart lists all of the readings that the patient has taken during their monitoring program. These readings can also include data entered manually by the Care Team.
The black triangle in the corner of the data square indicates that an intervention and/or a note was written for that reading. If you hover over the square, you will see a description of the alert condition. If the font of the number is yellow or red, it indicates that the alert has been accepted. If the background of the square is yellow or red, then the Active alert has not yet been accepted or rejected. You can also hover over any entry to see the date and time the entry was recorded as well as the time the entry was transmitted, if those times differ.
If the patient is assigned the RPM or +Go service level and is using a tablet/device to record their biometrics, they will be expected to use the Bluetooth devices provided. If an entry was added via direct entry in Care Team Portal, (d) will appear beside the value in the hover window. If it was entered manually on the tablet without a Bluetooth device, then (m) will appear beside the value in the hover window. If the patient enters the data manually too often, you see will an alert when you hover over the entry. You can review the trend for reporting Bluetooth or manual readings from the Patient Audit > Reading History tab. If a patient consistently reports their readings manually over several days, the patient may need assistance.

To view data in graph view, click .
If a patient has any alerts associated with their pathways or biometric readings, they will appear in the Alerts section. The Alerts section lists all of the alerts in the Program Trend area. After an alert is accepted or rejected, it will no longer appear in this section. You can accept or reject alerts in groups by clicking the Accept All or Reject All buttons or individually by clicking the row of the alert and then clicking the Accept or Reject button. The Audit view allows you to modify data that was previously accepted or rejected. You must have the Clinical Level 3 permissions assigned to your profile to modify entries.
To modify a biometric entry, click the square of the biometric you want to change, and then add a reason for modifying. Both accepting and rejecting require a note to be added for the reason you are making the modification.
- When you reject a previously accepted entry, the entry will appear with a line through it.
- When you accept a previously rejected alert, the line will be removed from the entry. When you return to grid view after rejecting an entry, the entry will no longer appear on the grid, but you can view it again using the Audit button.
To view a patient's biometrics data:
On the Patients page, find your patient and click in the center section of the Patient Header.
The Patient Monitoring page appears with the grid open. The biometric readings and survey responses are shown in the grid.
In the Program Trend area, click a bar in the chart to display the health index for that time period.
The font is yellow/red
The alerted data has been accepted. The background color of a cell is red or yellow
An Active alert that has not been accepted/rejected.
A black triangle symbol appears An intervention and/or note was written for that alert. (d) or (m) appears The entry was added via direct entry in Care Team Portal (d) or manually (m).
- Do any of the following:
- Click Graph. The biometric data appears in a line chart. A partial graph view is shown in the following graphic.
- Click Audit. If you have the appropriate permissions, you can strikeout or restore biometric readings.
- Click a point on a graph to shrink or expand the view.
