How To Create a New Patient Follow
To create a new patient in the Care Team Portal:
On the Patients page, click + Add Patient. The New Patient Profile page appears.
- Type the name of the new patient and other relevant patient data. Last Name is required. All other information is optional, but recommended for proper patient tracking.
- Click Save changes.
The Patient Profile page appears.
- On the Patient Profile, click Add Service Level to start the workflow for assigning a service level to the patient. The service level determines the type of technology the patient will use to participate in the monitoring program.
Add Special Handling, Emergency Contacts, and Providers.
