Managing Patient Pathways Follow
In the process of adding a new patient, Care Team members assign Pathways to their profile. Pathways are the questions and biometric prompts that are applied to a patient based on their health condition population. The Pathway questions and prompts appear on the patient’s device or IVR phone call during a scheduled time.
The Pathway is how you monitor a patient's health condition and track changes that occur during the duration of their program. You can configure the alerts for each question, so you will be informed when a patient's condition worsens or needs immediate attention. You can also add or remove questions and biometrics prompts based on the specific needs of each individual.
Some patients may require several Pathways to be applied to their profile due to multiple health conditions and others will have just one. You can adjust when a Pathway is sent to a patient by editing the Pathway schedule, which includes specifying the time the Pathway will become overdue and specifying when a patient can begin answering their questions.
For example, Johnathan Kent is an 84 year old man with Asthma. He has a RPM + Video service level, and his care team member assigned him the Asthma pathways to recur daily at 9:00 am. He is prompted every day to answer questions, such as "While at rest, how was your breathing yesterday" and "Did you take your daily maintenance inhaler." He is also prompted to record his biometrics for blood pressure, oxygen saturation percent, heart rate, and weight.
Note: To manage more advanced Pathway settings, such as creating new Pathway templates, you need the Content Administrator role assigned.
